General Purpose:

Provide direct nursing care to the residents and supervise the day-to-day nursing activities performed by nursing assistants. Such supervision must be in accordance with current Federal, State, and Local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing to maintain the highest degree of quality care at all times.

Essential Job Functions:

This facility expects their employees to promote an atmosphere of teamwork with other employees and hospitality and comfort for its residents. Therefore, the following list of duties is not all-inclusive:

Administrative Functions

  • Direct the day-to-day functions of the nursing assistants in accordance with rules, regulations, and guidelines that govern the long-term care industry.
  • Confirm that all nursing personnel assigned to you comply with the written policies and procedures established by this facility.
  • Meet with your assigned nursing staff, as well as support personnel, in planning the shift’s services, programs and activities.
  • Verify that all nursing personnel comply with the procedures set forth the in the Nursing Policies and Procedures Manual.
  • Make written and oral reports/recommendations concerning the activities of your shift, as required.
  • Cooperate with other resident services when coordinating nursing services be certain that the resident’s total regimen of care is maintained.
  • Verify that all nursing service personnel are in compliance with their respective job descriptions.
  • Participate in the development, maintenance, and implementation of the facility’s quality assurance program for the nursing department.
  • Participate in surveys (inspections) made by authorized government agencies.
  • Periodically review the resident’s written discharge plan. Participate in the updating of the resident’s written discharge plan, as required.
  • Assist in planning the nursing portion of the resident’s discharge plan, as necessary.
  • Admit, transfer, and discharge residents, as required.
  • Complete accident/incident reports, as necessary.

Charting and Documentation

  • Complete and file required forms/charts upon resident’s admission, transfer, and/or discharge.
  • Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies.
  • Receive telephone orders from physicians and record on the Physicians’ Order Form.
  • Transcribe physician’s orders to resident charts, cardex, medication cards, and treatment/care plans, as required.
  • Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident’s response to care.
  • Record new/changed diet orders. Forward information to the Dietary Department.
  • Report all discrepancies noted concerning physician’s orders, diet change, charting error, etc., to the Nurse Supervisor.
  • Fill out and complete transfer forms in accordance with established procedures.
  • Enter appropriate documentation concerning unauthorized discharges in the resident’s medical record in accordance with established procedures.
  • Perform routine charting duties as required and in accordance with established charting and documentation policies and procedures.
  • Sign and date all entries made in the resident’s medical record.
  • Write nurses’ notes to reflect that the care plan is being followed when administering nursing care or treatment.

Medication Administrative Functions

  • Prepare and administer medication as ordered by the physician.
  • Responsible for IV medication/fluid administration.
  • Verify the identity of the resident before administering the medication/treatment.
  • Verify that prescribed medication for one resident is not administered to another.
  • Verify that adequate supply of floor stock medications, supplies, and equipment is on hand to meet the nursing needs of the residents. Report needs to the Nurse Supervisor.
  • Order prescribed medications, supplies, and equipment as necessary, and in accordance with established procedures.
  • Verify that narcotic records are accurate for your shift.
  • Notify the Nurse Supervisor of all drug and narcotic discrepancies noted on your shift.
  • Review medication cards for completeness of information, accuracy in the transcription of the physician’s order and adherence to stop order policies.
  • Notify the attending physician of automatic stop orders prior to the last dosage being administered.
  • Immediately notify the Nurse Supervisor and attending physician of any medication administration errors.

Nursing Care Functions

  • Perform physical assessment through observation, inspection, palpitation, percussion and auscultation.
  • Deliver and maintain optimum resident care and comfort by demonstrating knowledge and skills of current nursing practices.
  • Promptly respond to call lights and other resident needs.
  • Make rounds with physicians, as necessary.
  • Consult with the resident’s physician in providing the resident’s care, treatment, rehabilitation, etc., as necessary.
  • Review the resident’s chart for specific treatments, medication orders, diets, etc., as necessary.
  • Make periodic checks to confirm that prescribed treatments are being properly administered by CNAs and to evaluate the resident’s physical and emotional status.
  • Notify the resident’s attending physician when the resident is involved in an accident or incident.
  • Notify the resident’s attending physician and responsible party when there is a change in the resident’s condition.
  • Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, giving massages and range of motion exercises, care for the dead/dying, etc., as required.
  • Monitor seriously ill residents, as necessary.
  • Review care plans daily to confirm that appropriate care is being rendered.
  • Confirm that CNAs are aware of the resident care plans. Verify that the CNAs refer to the resident’s care plan prior to administering daily care to the resident.

Other Duties

  • Attend in-service education programs in order to meet facility educational requirements.
  • Be familiar with Standard Precautions, Exposure Control Plan, Fire Drill and Evacuation Procedures and know how to use the information.
  • Maintain confidentiality of resident and facility records/information.
  • Protect residents from neglect, mistreatment, and abuse.
  • Protect the personal property of the residents of the facility.
  • Others as directed by the supervisor or administrator.

Minimum Qualifications:

  • Registered Nurse with current state license.
  • Excellent technical, assessment and documentation skills.
  • Good Leadership qualities.
  • Organized and detailed in work performance.
  • Good communication skills with excellent self-discipline and patience.
  • Genuine caring for and interest in elderly and disabled people in a nursing facility.
  • Comply with the Residents Rights and Facility Policies and Procedures.
  • Perform work tasks within the physical demand requirements as outlined below.
  • Perform Essential Duties as outlined above.

Job Summary:

Plan and administer a prescribed occupational therapy treatment program for patients/residents to facilitate 1·ehabilitation following illness, disease or injury

Responsibilities:

  1. Review and evaluate physician referrals and patient / resident medical records to determine occupational therapy treatment required.
  2. Plan and prepare written treatment program based on evaluation of available patient / resident data.
  3. Confer with physician and other health practitioners to obtain additional patient/resident information, suggest revisions in treatment program, and integrate therapy treatment ·with other aspects of patient / resident care.
  4. Record patient/resident treatment, response and progress.
  5. Assist the rehab director and medical director in planning of occupational therapy services as needed.
  6. Perform tests, measurements and evaluations such as functional activities/ADI.s, range of motion, manual muscle tests, postural control, sensation, coordination, neuromuscular control, visual perception and home management.
  7. Plan program involving practice in functional skills including prevocational, vocational and homemaking skills and activities of quality living.
  8. Consult ·with other members of the rehabilitation team to select programs consistent with needs and capabilities of each patient and to coordinate occupational therapy with other therapeutic activities.
  9. Select treatment program suited to individual physical capacity, intelligence level and interest to upgrade patient to maximum independence, assist in restoration of functions and aid in adjustment to disability.
  10. Consult with physicians and/or nursing in the event of adverse reactions.
  11. Maintain documentation consistent with third party reimbursement.
  12. Conduct on the job training for new employees. Adapt existing training materials to meet immediate needs.
  13. Assist in the recruitment of staff to provide needed treatment.
  14. Attend department meetings and/or sit on required committees.

Pharmacist Clinical 
*Selected locations preferred. Candidate must be willing transfer back to working out of Ingenio hub office once notified.

Summary Requirements: 

  • Researches and synthesizes detailed clinical data related to pharmaceuticals.
  • May review and approve or deny coverage for pharmaceuticals (as permitted by state/federal law or state/federal program contracts) based on medical necessity criteria, and coordinates with Anthem or health plan medical directors as needed.
  • Serves as a clinical resource to other pharmacists on areas such as prospective and retrospective DURs and provides dosage conversion and clinical support for therapeutic interventions.
  • Prepares information for network physicians.
  • Primary duties may include, but not limited to:
  • Experience in Injectable/Specialty Drug oversight including Drug Utilization Management.
  • Make prior authorization determinations based upon evidence-based clinical data.
  • Prior authorization determinations, research, and synthesize detailed clinical data related to injectable pharmaceuticals.
  • This position will coordinate with the Medical Directors and RNs in the UM Department, Providers. 
  • Provides evaluation of member Rx needs and therapies through review of medication history to determine if medications are appropriate.
  • Provide injectable drug medical necessity reviews covered under the Medical benefit (Part B).
  • Determines potential barriers to compliance and identifies solutions to overcome barriers.
  • Adhere to all Medicare and policy and procedures. 
  • Ensures that providers understand appropriate use of current prescriptions.
  • Reviews medication requests with physicians and/or physician representatives when therapies do not meet national standards.
  • Serves as a resource to the clinical team in disease management and other clinical areas.

 

Qualifications

Qualifications:    

  • Must be a registered pharmacist; minimum 2 years of managed care pharmacy (PBM) experience or residency in lieu of work experience; or any combination of education and experience, which would provide an equivalent background
  • Must possess an active unrestricted state license to practice pharmacy as a Registered Pharmacist (RPh).
  • PharmD preferred.
  • Unless expressly allowed by state or federal law, or regulation, must be located in a state or territory of the United States when conducting utilization review or an appeals consideration and cannot be located on a US military base, vessel or any embassy located in or outside of the US, unless they are command-sanctioned activities.
  • Requires strong oral, written and interpersonal communication skills, problem-solving skills, facilitation skills, and analytical skills.
  • Experience with direct patient counseling, provider interaction and/or education preferred.
  • Understanding of the Specialty Drug market preferred.
  • Clinical knowledge of Biotech/Oncology drugs preferred.
  • Part B experience strongly preferred.
  • Must have excellent PC skills (Word, Outlook, Excel).
  • Clinical pharmacy experience strongly preferred.
  • This position is for 2nd shift coverage Monday through Friday 1pm to 10pm with weekend and holiday rotation.

As an extension of the primary care physician’s (PCP) care team, RN Care Managers are responsible for providing a variety of Care Management services within a PCP practice(s) targeting patients identified as high risk and/or those who are experiencing barriers to meeting their healthcare goals. Principle Care Management services include, but are not limited to, performing comprehensive assessments, developing patient-centered care pans, providing episodic and longitudinal care planning. RN Care managers also monitor acute facility stays and discharges, provide disease education and empower patient’s ability to develop self-management skills.

How you can make a difference

  • Actively engage and collaborate with PCP’s and office staff in identifying high-risk patients
  • Employ motivational interviewing skills to elicit optimal member engagement/outcome
  • Perform comprehensive assessments for both physical and psychosocial risk factors that support individual patient needs while identifying and addressing barriers
  • Communicate assessment findings, care plan goals, interventions and outcomes to PCP, patients and caregivers in a timely manner
  • Monitor patient’s acute stays, perform post-discharge follow up calls and continuously assess risk of readmissions post-discharge
  • Identify and support practice needs for structured on-site Care Coordination presence in alignment with program model
  • Maintain a core understanding of population management as it specifically relates to high risk patients
  • Provide chronic disease education and symptom management teaching to patients and caregivers
  • Assess medication adherence and perform comprehensive medication reconciliation
  • Address Gaps in Care for High Risk patients engaged in Care Management services
  • Document clinical interventions in applicable care management software systems
  • Develop and maintain effective professional working relationships with assigned PCP practice(s)
  • Engage patients in a variety of settings, determined by program models and initiatives

Experience to drive change

  • 3+ years of direct, clinical nursing experience
  • Registered Nurse with licensure in the state of practice
  • Care management experience in a setting that requires assessment, critical thinking and application
  • Comfort with technology including Microsoft suite of products
  • Utilizing a variety of electronic health records including data capture, data mining and reporting

  • Evaluates emergency situations and initiates appropriate nursing interventions. Leads the nursing response to medical emergencies. Ensures the readiness of the Virtual Care Team members to respond to emergencies, including appropriate resources and staff knowledge and skill.
  • Directs other clinical support staff in the delivery of patient care.
  • Assesses patient and family health education needs, provides health teaching and counseling and evaluates effectiveness of the education process.
  • Accurately documents patient data, actions, interventions and patient response according to established guidelines and policies.
  • Leads or participates in quality improvement activities and/or initiatives.
  • Serves as an advocate for the patient. Facilitates transitions and coordination of care across the continuum.
  • Serves as a role model for quality and service and as a resource, coach and mentor for other members of the Health Care Team.
  • Completes other department-specific procedures and nursing care within scope of practice and skill.

    The Registered Nurse (RN) is a proactive member of an interdisciplinary team of licensed and unlicensed care givers who ensure that patients, families and significant others receive individualized high quality, safe patient care. They practice in a clinical environment that is administered by Nurse Managers and other leaders and is supported through the WellStar
    Shared Governance Model. The framework for practice is steered by the WellStar Professional Practice Model, 5-Star Nursing and evidence based practice and research. RN competencies are derived from these models and supported by the WellStar Core Competencies.

    Qualifications


    Required Minimum Education: Graduate of an accredited/approved school of nursing


    Preferred Education: Baccalaureate degree in nursing (BSN) from an accredited school of nursing preferred


    Required Minimum License: Currently licensed as a Registered Nurse in the State of Georgia or hold a privilege to practice in the State of Georgia under the Enhanced Nurse Licensure Compact (eNLC).


     Required Minimum Certification: Must have a current BLS card from the American Heart Association on the first day of employment in position. Must obtain ACLS( Advanced Critial Life Support) certification within 6 months of employment.

     

    Preferred Experience: Experience with direct patient care preferred


    Required Minimum Skills: Ability to read, write and speak English language, optimize the use of technology to support clinical care and holds basic computer skills; Strong interpersonal, collaborative skills along with customer service skills required; Ability to function in a fast paced environment and respond to emergencies in using a decisive, composed and respectful manner; Possess excellent time management skills; practices nursing using evidence and analytical skills along with possessing strong critical thinking skills.
