General Purpose:

Occupational therapy assistants provide services for treating impairments, functional limitations, disabilities or changes in physical function and health status under the direction and supervision of the treating occupational therapist and in accordance with the patient’s plan of care, accepted standards of professional practice, applicable law and regulation, as well as applicable Interim HealthCare policies, procedures and clinical pathways.

Essential Functions:

  • Participates in ongoing interdisciplinary assessment of the patient.
  • Acts at the direction and under the supervision of the treating occupational therapist and in accordance with the plan of care.
  • Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
  • Provides services that are ordered by the physician as indicated in the plan of care.
  • Assists the treating occupational therapist in documenting a patient’s progress and response to treatment.
  • Teaches task-oriented therapeutic activities designed to restore physical function, as well as sensory-integrative function,
  • Teaches compensatory technique to improve the level of independence in the activities of daily living (ADLs).
  • Completes and submits timely, legible, accurate, relevant, and sufficient clinical documentation (written or electronic) addressing the treatment and interventions provided, the patient’s response to the treatment and interventions, and the patient’s progress toward goals in accordance with professional standards of practice, policy and procedures, and payor requirements.
  • Counsels and educates the patient and the family, and other caregivers to promote patient progress toward mutually established goals and in meeting healthcare related needs.
  • Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient care and planning for discharge
  • Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
  • Participates in Quality Assessment and Performance Improvement (QAPI) program.
  • Participates in in-services or training, as requested.
  • Participates in the Office’s Emergency Preparedness program, as requested.
  • Completes other assignments as requested and assigned.
  • Accesses personal health information (“PHI”) necessary to fulfill the above duties and responsibilities.  Access to use and ability to disclose PHI is further defined by each organization/department.

Licensure, Minimum Education & Experience Requirements:

  • Current GA OTA licensure in good standing
  • A graduate of an occupational therapy education program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association (AOTA), or its successor organizations of ACOTE and is eligible to take, or has successfully completed the entry-level certification examination for occupational therapy assistants developed and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT).
  • In the last five (5) years, two (2) years of experience as an occupational therapy assistant that is relevant to the patient population assigned preferred.

Knowledge, Skills & Abilities Required: 

  • Current CPR certification
  • Current negative PPD or chest x-ray 
  • HCHB charting experience preferred 
  • Valid Drivers License and Auto Insurance

Responsibilities


The Care Partner (CP) is a support care provider who functions under the direction of the clinical nurse (Registered Professional Nurse). The CP primary focus will be to promote a positive patient experience by partnering with the patient, family and other health care providers to provide excellence in customer service and assistance with daily living activities and comfort measures. The CP assists other health care providers with safe, age appropriate and culturally aware care by performing basic patient care functions; i.e. ADL’s, ambulation, vital signs, approved advanced technical procedures; i.e. phlebotomy, simple treatments, dressing changes, oral suctioning, and discontinuing foley catheters; transporting supplies/equipment/patient medications. May be required to support clinical care by performing clerical functions including computer order entry, retrieval of data, maintaining unit supplies, transporting supplies/equipment/patient medications, preparing and maintaining patient charts, providing customer service and acting as the central communication source on the nursing unit and other activities as necessary. The CP must be able to work in a fast paced environment which requires multi-tasking activities and ongoing communication with the registered nurse. Must be able to perform under stressful conditions with a compassionate, respectful and helpful manner. Must be able to take direction from a variety of care providers. Must be organized with excellent communication skills.


Qualifications


Required Minimum Education

  • High school diploma or equivalent from accredited program required


Required Minimum Experience

  • Six (6) months clinical experience in acute care setting, long-term care facility,physician office/clinic, or other direct clinical patient care experience or minimum one (1) semester clinical in an Associate or Baccalaureate degree nursing program preferred.
  • One year experience and/or certification as a Medical Assistant preferred
  • Ability to enter information into an electronic medical record
  • Basic medical terminology required


Required Minimum Certification

  • Basic Life Supoort (BLS) certification through American Heart Association (AHA) is required.\


Required Skills

  • Ability to read, write and speak the English language
  • Basic computer skills required
  • Strong customer service and interpersonal skills

  • Current LPN license, with no sanctions, restrictions, or provisions, in the state of Georgia.

  • One year of related experience in a clinical environment

General Purpose:

Provide direct nursing care to the residents and supervise the day-to-day nursing activities performed by nursing assistants. Such supervision must be in accordance with current Federal, State, and Local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing to maintain the highest degree of quality care at all times.

Essential Job Functions:

This facility expects their employees to promote an atmosphere of teamwork with other employees and hospitality and comfort for its residents. Therefore, the following list of duties is not all-inclusive:

Administrative Functions

  • Direct the day-to-day functions of the nursing assistants in accordance with rules, regulations, and guidelines that govern the long-term care industry.
  • Confirm that all nursing personnel assigned to you comply with the written policies and procedures established by this facility.
  • Meet with your assigned nursing staff, as well as support personnel, in planning the shift’s services, programs and activities.
  • Verify that all nursing personnel comply with the procedures set forth the in the Nursing Policies and Procedures Manual.
  • Make written and oral reports/recommendations concerning the activities of your shift, as required.
  • Cooperate with other resident services when coordinating nursing services be certain that the resident’s total regimen of care is maintained.
  • Verify that all nursing service personnel are in compliance with their respective job descriptions.
  • Participate in the development, maintenance, and implementation of the facility’s quality assurance program for the nursing department.
  • Participate in surveys (inspections) made by authorized government agencies.
  • Periodically review the resident’s written discharge plan. Participate in the updating of the resident’s written discharge plan, as required.
  • Assist in planning the nursing portion of the resident’s discharge plan, as necessary.
  • Admit, transfer, and discharge residents, as required.
  • Complete accident/incident reports, as necessary.

Charting and Documentation

  • Complete and file required forms/charts upon resident’s admission, transfer, and/or discharge.
  • Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies.
  • Receive telephone orders from physicians and record on the Physicians’ Order Form.
  • Transcribe physician’s orders to resident charts, cardex, medication cards, and treatment/care plans, as required.
  • Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident’s response to care.
  • Record new/changed diet orders. Forward information to the Dietary Department.
  • Report all discrepancies noted concerning physician’s orders, diet change, charting error, etc., to the Nurse Supervisor.
  • Fill out and complete transfer forms in accordance with established procedures.
  • Enter appropriate documentation concerning unauthorized discharges in the resident’s medical record in accordance with established procedures.
  • Perform routine charting duties as required and in accordance with established charting and documentation policies and procedures.
  • Sign and date all entries made in the resident’s medical record.
  • Write nurses’ notes to reflect that the care plan is being followed when administering nursing care or treatment.

Medication Administrative Functions

  • Prepare and administer medication as ordered by the physician.
  • Verify the identity of the resident before administering the medication/treatment.
  • Verify that prescribed medication for one resident is not administered to another.
  • Verify that adequate supply of floor stock medications, supplies, and equipment is on hand to meet the nursing needs of the residents. Report needs to the Nurse Supervisor.
  • Order prescribed medications, supplies, and equipment as necessary, and in accordance with established procedures.
  • Verify that narcotic records are accurate for your shift.
  • Notify the Nurse Supervisor of all drug and narcotic discrepancies noted on your shift.
  • Review medication cards for completeness of information, accuracy in the transcription of the physician’s order and adherence to stop order policies.
  • Notify the attending physician of automatic stop orders prior to the last dosage being administered.
  • Immediately notify the Nurse Supervisor and attending physician of any medication administration errors.

Nursing Care Functions

  • Deliver and maintain optimum resident care and comfort by demonstrating knowledge and skills of current nursing practices.
  • Promptly respond to call lights and other resident needs.
  • Make rounds with physicians, as necessary.
  • Consult with the resident’s physician in providing the resident’s care, treatment, rehabilitation, etc., as necessary.
  • Review the resident’s chart for specific treatments, medication orders, diets, etc., as necessary.
  • Make periodic checks to confirm that prescribed treatments are being properly administered by CNAs and to evaluate the resident’s physical and emotional status.
  • Notify the resident’s attending physician when the resident is involved in an accident or incident.
  • Notify the resident’s attending physician and responsible party when there is a change in the resident’s condition.
  • Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, giving massages and range of motion exercises, care for the dead/dying, etc., as required.
  • Monitor seriously ill residents, as necessary.
  • Review care plans daily to confirm that appropriate care is being rendered.
  • Confirm that CNAs are aware of the resident care plans. Verify that the CNAs refer to the resident’s care plan prior to administering daily care to the resident.

Other Duties

  • Attend in-service education programs in order to meet facility educational requirements.
  • Be familiar with Standard Precautions, Exposure Control Plan, Fire Drill and Evacuation Procedures and know how to use the information.
  • Maintain confidentiality of resident and facility records/information.
  • Protect residents from neglect, mistreatment, and abuse.
  • Protect the personal property of the residents of the facility.
  • Others as directed by the supervisor or administrator.

Minimum Qualifications:

  • Licensed Practical Nurse with current state license.
  • Excellent technical, assessment and documentation skills.
  • Good Leadership qualities.
  • Organized and detailed in work performance.
  • Good communication skills with excellent self-discipline and patience.
  • Genuine caring for and interest in elderly and disabled people in a nursing facility.
  • Comply with the Residents Rights and Facility Policies and Procedures.
  • Perform work tasks within the physical demand requirements as outlined below.
  • Perform Essential Duties as outlined above.

  • Prepare treatment room with strict adherence to safety protocols and following OSHA & CDC recommendations
  • Manage supply and inventory levels, as well as place and expedite orders as needed
  • Partner with the providers and team to implement Heartland Dental systems and maximize office workflow

DaVita is seeking a Registered Nurse who is looking to give life to patients in an Acute Hospital setting. RNs in this role work autonomously alongside the hospital staff to assess, troubleshoot, and make sound decisions for the critically ill patients.

If you haven’t considered Nephrology nursing before, read on as we think that you should. Dialysis Experience is NOT required.

What you can expect:

  • Use your critical care thinking skills. Whether this is the first or a blip in their chronic treatment, our RNs are experts in educating our patients and families. You’ll be able to leverage your nursing skills for patients who are often dealing with multiple co-morbidities and/or acute conditions which require unique treatment plans and the ability to leverage a broad range of nursing skills and knowledge.
  • Not just hemodialysis. Acute RNs may deliver nephrology care across multiple renal therapies: peritoneal, continuous renal replacement and apheresis.
  • Autonomy in a hospital setting. Deliver dialysis to patients at a particular hospital or group of hospitals and become an integral part of their team and ours. As a teammate in an acute setting you work on your own almost all of the time. Each day you assess, troubleshoot, and make sound decisions for critical patients. Our Hospital Services group performs 1.1M treatments annually.
  • Dynamic environment. No two days are ever the same for an acute dialysis nurse. You’ll work within a variety of settings (ICU, CCU, ER and more), and you’ll be able to keep learning new nursing skills on top of dialysis.
  • Schedule: Provide emergent, lifesaving care during on call shifts. Schedules vary by hospital system. For this role expect to know your weekly schedule in advance, but your daily schedule may change depending on patient needs. Must be in commutable distance based on on-call requirements.
  • The Gold Seal. As an Ambulatory Health Care accredited provider, DaVita is following and meeting the highest standards of care, providing care in line with the highest industry standards according to The Joint Commission. We expect our nurses to commit to improving patient health through clinical goal setting and quality improvement initiatives.

Some additional details about this position:

  • Training may take place in a facility or a training clinic with a class of new teammates other than your assigned home clinic
  • Hours of work may vary during the training period; however, you will typically be working between 32-40 hours per week during training
  • You must have a flexible schedule and be able to work mornings, evenings, weekends, and holiday

You Must Have (aka. Requirements):

  • Current and valid Registered Nurse (RN) license in the state where practicing
  • Willingness to work a flexible schedule with mandatory overtime required due to the nature of the acute treatment you are providing.
  • Work long days without notice
  • Ability to travel to multiple hospitals within a given area (Be sure to ask your recruiter about the commute range for this position.)
  • Experience assessing, trouble shooting and making sound recommendations in stressful situations
  • Physical Requirement: nurses often have to push/pull large hemodialysis machines around the hospital with little or no assistance
  • Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree
  • Basic computer skills and proficiency in MS Word and Outlook

You might also have (a.k.a. preferred, not required):

  • Hemodialysis nursing experience in an acute or chronic setting
  • Medical/surgical (Med/Surg), Intensive Care Unit (ICU), Critical Care Unit (CCU), and/or Emergency Room (ER) experience
  • Certified Nephrology Nurse (CNN), Certified Dialysis Nurse (CDN) and/or Certified Hemodialysis Nurse (CHN)

The Loan Ops Technician II is responsible for comprehensive knowledge of the loan operation functions, including Commercial, Construction, Consumer Loans, and Lines of Credit. This includes, but is not limited to, onboarding of new, renewed, and modified loans onto Core System, processing loan maintenance requests, exception item processing, payment and payoff processing, various reports, and servicing of specialty lending products such as Participations, Syndications, SWAP loans, Escrow and Insurance. This position also assists with special initiatives and supports other areas within Loan Operations as needed while demonstrating the Ameris Vision, Purpose, Expectations and Values. Essential Functions, Duties, and Responsibilities:

· Strictly adheres to Bank Secrecy Act requirements and applies organizational policies and procedures in all activities.

· Performs necessary actions for onboarding new, renewed, and modified loans onto the Core System (FIS IBS).

· Perform complex loan servicing tasks on loans including, but not limited to, exception item processing, payment and payoff processing, account maintenance, insurance and escrow processing, and servicing of specialty lending products.

· Prepares and submits filings, reports and related documents.

· Uses tickler system and software applications to track, monitor, update and maintain status and records.

· Process all work orders, incoming requests and payments within defined SLAs and timeframes.

· Assembles loan files, verifies accuracy and completion of records, and executes storage procedures.

· Responds to customer inquiries and requests.

· Maintains compliance with internal controls, operational procedures and risk management policies.

· Assist with review and updates to departmental procedures and assist management with process improvements.

· Review and maintain various departmental reports in order to identify trends or areas of concern.

· Works to understand the root causes of customer service issues. Identifies and implements resolutions for routine issues involving supervisors on more complex issues.

· Performs account calculations for verification.

· Performs quality assurance audits.

· Completes compliance training, adheres to all anti-money laundering guidelines and procedures, and all regulatory requirements.

· Perform other related job duties and special projects as assigned by Management.

Required Knowledge, Skills and Competencies:

· Computer and Microsoft Office proficiency.
