Responsibilities


The Rapid Response Team (RRT) Nurse is a registered professional nurse who functions under the direction of the Nurse Manager and is responsible for coordination of Rapid Response Team assessment of patient, prioritization and implementation of interventions to ensure optimal patient outcomes, documentation of interventions and outcomes, communication with other team members and disciplines, and safe delivery of age appropriate and culturally aware patient care. The RRT Nurse serves as a resource/support to the staff, physicians, patients, families, and other departments. Qualifications


Required Minimum Education: Graduate of an accredited/approved school of nursing;

 

Required Minimum Licensure:  Current licensure as a professional registered nurse in the State of Georgia; Baccalaureate degree in nursing (BSN) preferred.

Responsibilities


The Care Partner (CP) is a support care provider who functions under the direction of the clinical nurse (Registered Professional Nurse). The CP primary focus will be to promote a positive patient experience by partnering with the patient, family and other health care providers to provide excellence in customer service and assistance with daily living activities and comfort measures. The CP assists other health care providers with safe, age appropriate and culturally aware care by performing basic patient care functions; i.e. ADLs, ambulation, vital signs, approved advanced technical procedures; i.e. phlebotomy, simple treatments, dressing changes, oral suctioning, and discontinuing Foley catheters; transporting supplies/equipment/patient medications. May be required to support clinical care by performing clerical functions including computer order entry, retrieval of data, maintaining unit supplies, transporting supplies / equipment/patient medications, preparing and maintaining patient charts, providing customer service and acting as the central communication source on the nursing unit and other activities as necessary. The CP must be able to work in a fast paced environment which requires multi-tasking activities and ongoing communication with the registered nurse. Must be able to perform under stressful conditions with a compassionate, respectful and helpful manner. Must be able to take direction from a variety of care providers. Must be organized with excellent communication skills.


Qualifications


Required Minimum Education
  • High school diploma or equivalent from accredited program
Required Minimum Experience
  • Six (6) months clinical experience in acute care setting, long-term care facility, physician office/clinic, or other direct clinical patient care experience or minimum one (1) semester clinical in an Associate or Baccalaureate degree nursing program.
  • Ability to enter information into an electronic medical record
  • Basic medical terminology

Preferred Minimum Experience

  • One year experience and/or certification as a Medical Assistant 
Required Minimum Certification
  • Basic Life Support (BLS) certification through American Heart Association (AHA) is required by the first day of employment.
Required Skill
  • Ability to read, write and speak the English language
  • Basic computer skills; Strong customer service and interpersonal skills

  • Responsible for adhering to all government regulations and company standards
  • Guide parents and patients through our entire treatment process
  • Assist with dental and perio charting; when necessary
  • Take x-rays and save into patient charts, when necessary

Qualifications

Requirements:

  • Current licensure as a Dental Hygienist in good standing
  • Experience is great, but not always required
  • Speak another language? That may be very helpful here.

Overview

Responsibility to function as an integral member of the health care team through the provision of medical care of patients and through participation in the evaluation of center operations to ensure an effective and efficient medical care delivery system.

Qualifications
  • Medical Doctor (MD) or Doctor of Osteopathic (DO) medicine degree from an accredited medical school.
  • Board Certified or Board Eligible in Family Medicine.
  • Current, unrestricted license to practice medicine in the state of Georgia.
  • Must have current Drug Enforcement Agency (DEA) registration.
  • Current Healthcare Provider Cardiopulmonary Resuscitation (CPR) course completion card.
  • Current Basic Life Support (BLS) certification for healthcare providers.
  • Experience in working in a community health center is desirable but a high level of interest in community medicine is acceptable in lieu of experience.
  • Knowledge and understanding of the health care norms and pathology in practice as a Family Medicine Practitioner and the ability to apply this knowledge in direct patient care.
  • Excellent communication, organizational and customer service skills.
  • Excellent phone etiquette.
  • Detailed oriented and efficient required.
Responsibilities
  • Provides primary health care through examination and treatment of patients at the Center and in certain instances, through satellite offices.
  • Admits and treats patients in the hospital(s) indicated based on area of medical practice.
  • Refers patients for specialty care as necessary and through use of support staff and assures that such care is received.
  • Rotates on-call for emergencies or as scheduled in the evenings and on weekends as scheduled by the Medical Director and assure continuity of care for patients under his/her care.
  • Comply with Quality Assurance policies and procedures as developed by the health center’s Quality Assurance committee.
  • Work with the Medical Director in developing the Health Care Plan implementing clinical policies and procedures and maintaining a superior quality of patient care.
  • Work with Center Administration to ensure the financial well-being of the center, in particular completing necessary paperwork pertaining to a patient’s visit to ensure proper billing, participating in and providing feedback on revenue maximization, cost containment, operational efficiency and program effectiveness.
  • Other medical duties may be required from time to time as a result of the Center’s participation in the primary care network or affiliated managed care entities.

  Summary:  Assess each patient, develop a plan of care, safely implement and modify plans as necessary in accordance with American Physical Therapy Association Code of Ethics and Standards of Practice, American Physical Therapy Association's Guide to Physical Therapists Practice and State Practice Act.

 

Responsibilities

Responsibilities:

  • Upon physician referral, conduct an initial patient evaluation to develop an appropriate treatment plan with measurable goals and time frames.
  • Document treatments and provide documentation per policy and procedure.
  • Ensure that other treating medical professionals have the necessary information to appropriately evaluate the patient's functional ability.
  • Communicate on a regular basis with all therapy patients, setting their expectations for return-to-work and emphasizing education and motivation for compliance with the established treatment plan(s).
  • Meet with the Market Director of Physical Therapy and or other staff on a regular basis, including off-site to discuss quality of patient care, procedures, records, review of policies, attend training sessions, educational seminars and other issues as required.
  • Partner with the Market Director of Physical Therapy and Medical Center Administrator (MCA) to ensure center operations are consistent with medical policy and professional standards at Nova Medical Centers.
  • Provide input to the Regional Director of Physical Therapy on capital equipment proposals, additional PT support staff, and etc. to ensure responsibilities are consistently met.
  • Coordinate with MCA in order to eliminate missed appointments, as well as, ensures that center support staff follows up on missed appointments the same day the appointment was missed.
  • Promote, support and engage in projects, communications and activities which are designed to maintain, develop and/or improve overall teamwork within the clinic. Assist with developing and promoting clear communication between the Physical Therapy department and other center staff members.
  • Assure compliance with licensing, certification and accrediting bodies, as well as remain familiar with rules and regulations established by relevant licensing bodies.
  • Communicate effectively with a diverse patient population.

Qualifications

Qualifications:

  • Graduate of an accredited school of Physical Therapy
  • Current, or ability to obtain, licensure with the State Board of Physical Therapy Examiners
  • CPR Certified or ability to obtain
