Urgent Care Nurse Practitioner/Physician Assistant
Provides primary and urgent care to patients following accepted standards and protocols.
Primary Responsibilities:
- Perform complete physical exams and assessments of patients including urgent and non-urgent presentation.
- Select, order, perform and interpret tests, analyses and diagnostic images to provide information on patient condition and augment physical findings.
- Analyze reports and findings of tests to diagnose patients.
- Prescribe or administer treatment, therapy, medication, vaccination and other specialized medical care to treat or prevent illness and injury.
- Perform procedures including laceration repair, orthopedics and provide referrals to medical specialists.
- Explain procedures and discuss test results and prescribed treatments with patients.
- Notate findings and treatment course in patient electronic chart including follow-up notes.
- Perform physicals—including sports, DOT, non-DOT, pre-employment and annuals.
- Evaluate patients for workers’ compensation injuries, coordinating with employer/clients to balance the needs of the patient and requirements of the employer.
- Oversee mid-level providers working independently at clinics within the region, when necessary.
Key Requirements:
- 2+ years clinical experience providing patient care as PA, NP or Staff Physician. Preference for ABMS Board certification in internal medicine, family practice, or emergency medicine.
- Understanding of medical coding and billing.
- Knowledge of state and federal regulations including OSHA, HIPAA, blood borne pathogens and others.
- Competent with common PC applications including Internet, Email and Microsoft Office as well as comfortable with EMR applications.
- Understanding and experience with occupational health practices, including CME qualification, drug testing protocols, employment physicals, and OSHA guideline.

Responsibilities:

Within your first 90 days at Virta, we expect you will do the following:
- Successfully take on and manage both new, current, and tenured patients; roughly fifteen new patients per week.
- Partner with our existing health coaches to learn and understand the entire patient experience, from enrollment to daily patient care and long-term patient success.
- Shadow current health coaches and learn how to successfully perform patient onboarding calls and day to day patient care.
- Shadow and learn from current health coaches the various platforms and tools needed to perform your day to day job.
- Learn and understand what key performance indicators (KPIs) are used to track health coach performance and how your day to day work impacts those metrics.
- Actively contribute to team culture, learning, and overall process improvements by providing regular continuous feedback.

The Care Coordinator provides intensive case management services for consumers with severe and persistent behavioral health disorders. Typically, the goal is prevention of hospitalization or other out-of-home placements. The Care Coordinator provides services primarily in community settings: home, school and other service settings.
The Care Coordinator conducts face-to-face comprehensive assessments to identify problems and service needs. Care Coordinator will utilize their personal recovery experience as a tool in providing support to consumers in meeting daily recovery goals and during crisis episodes. Care Coordinator will function as part of an interdisciplinary team to provide indirect and direct clinical and supportive services. Provides and bills only those consumer services that are authorized in the service plan.
Arranges appointment schedule at optimal intervals to maximize use of work time. Monitors consumer progress and compliance with the service plan. Writes timely progress notes that relate directly to the treatment outcomes described in the service plan. Assembles, analyzes and interprets new/updated information about the consumer in order to reassess treatment needs. Identifies unmet needs, reviews available resources, and modifies the treatment plan as indicated. Requests treatment authorizations and re-authorizations within required time frames.
Provides service linking and monitoring. Establishes linkages and coordination of services with community agencies. Develops and maintains cooperative working relationships with community service providers. Informs consumers and families of the availability of community advocacy and support groups.
Other critical services provided include symptom management, behavior management planning, teaching of relevant skills, supportive community-based interventions, and crisis intervention services. The community Care Coordinator participates in interdisciplinary case staffing and staff meetings. Program
Description: Intensive Case Management (ICM) uses a multidisciplinary team approach to provide intensive mental health treatment services to individuals with severe and persistent mental illness in their homes and other community settings-24 hours a day, seven days a week, 365 days a year. ICM is intensive, and its goal is to keep individuals out of state hospitals, jails, emergency rooms and other institutions. ICM clients have to meet a strict criterion (i.e. diagnosed with a severe and persistent mental illness, unable to maintain activities of daily living, and be unsuccessful in other forms of treatment). ICM clients often have no resources and no family support and require an intensive and expeditious form of treatment.
Conduct one home visit per week/per client within their community setting or within client’s current residence.
Develops and maintains a network of community contacts. Facilitates referrals and linkages to community services and communication among client’s service providers with VPH to assure a continuum of services.
· Provide linkage, transport clients, and assist/coach clients in accessing & obtaining community resources.
· Establishes linkage with community resources (jails, hospitals, shelters, DFCS, etc.) to assist with discharge planning and a smooth transition to View Point Health Services.
· Process referrals from other inter-agency providers & Community partners & check against strict requirements for ICM services for acceptance or denial.
· ICM providers are expected to carry an individual case load of 20 to 30 clients.
· Write precise, accurate, and clear progress notes in our electronic Care Logic system in a timely manner. Maintain required records and information in each client’s file. Prepares accurate, complete timely documentation (within 24 hours) of case notes and billing information to meet agency and state standards and provide effective service delivery across a broad range of services.
· Provide individual skills training as indicated in the client’s treatment plan including but not limited to independent living skills, coping skills, interview skills, symptom management skills, anger management skills, assertiveness skills, advocacy skills, hygiene skills, and many other daily living skills.
· Recognize the need for crisis intervention and notifies supervisor and professional staff of escalated situations.
· Under professional guidance, provides individual and group supportive counseling to clients and families concerning their needs and problems, and issues with personal and social development.
- Work afterhours and on weekends when needed

Under general supervision, provides Tutoring to students in the Academic Success Center for the purpose of providing tutorial services in Math, English/Reading, Anatomy, Physiology and Biology. Demonstrates the use of appropriate teaching techniques. Provides positive feedback. Discuss student progress with appropriate staff/faculty. Maintains a positive, warm attitude that encourages the student(s) to learn. Demonstrates the use of appropriate testing and grading procedures including proper maintenance of grade books and any other appropriate record-keeping required. Demonstrates the effective use of oral and written communication skills. Follows approved course syllabus. Completes all reports, records and invoices in a timely manner and maintains an accurate inventory of all assigned properties. Advises and counsels students and maintains appropriate advisement records on each student. Maintains classrooms and labs in accordance with institutional safety guidelines. Attends meetings as required and appropriate. Observes and enforces the institution’s policies and regulations. Maintains qualifications for employment in accordance with institutional and accreditation guidelines. Maintains qualifications for employment as described by the Technical College system of Georgia Policy Manual. Follows rules and regulations as described by the Technical College System of Georgia Policy Manual and other policies and procedures established by the institution. Employment has to be renewed each semester.
MINIMUM QUALIFICATIONS
Current technical college student with an overall GPA of 3.0 and a minimum GPA of 3.0 in the subject area to be tutored from a regionally accredited institution *or* outside applicant that demonstrates the ability and experience in the subject field.
PREFERRED QUALIFICATIONS
Associate level graduate or beyond with an overall GPA of 3.0 and a minimum GPA of 3.0 in the subject area to be tutored.
WORK SCHEDULE
8am-12pm, 12pm-3pm and 3pm-8pm - 19.5 hours per week
