LICENSURE AND EMPLOYMENT OF PERSONS PREVIOUSLY CONVICTED OF ONE OR MORE CRIMINAL OFFENSES

Section 750. Definitions.

751. Applicability.

752. Unfair discrimination against persons previously convicted of one or more criminal offenses prohibited.

753. Factors to be considered concerning a previous criminal conviction; presumption.

754. Written statement upon denial of license or employment.

755. Enforcement.

§750. Definitions. For the purposes of this article, the following terms shall have the following meanings:

(1) "Public agency" means the state or any local subdivision thereof, or any state or local department, agency, board or commission.

(2) "Private employer" means any person, company, corporation, labor organization or association which employs ten or more persons.

(3) "Direct relationship" means that the nature of criminal conduct for which the person was convicted has a direct bearing on his fitness or ability to perform one or more of the duties or responsibilities necessarily related to the license, opportunity, or job in question.

(4) "License" means any certificate, license, permit or grant of permission required by the laws of this state, its political subdivisions or instrumentalities as a condition for the lawful practice of any occupation, employment, trade, vocation, business, or profession. Provided, however, that "license" shall not, for the purposes of this article, include any license or permit to own, possess, carry, or fire any explosive, pistol, handgun, rifle, shotgun, or other firearm.

(5) "Employment" means any occupation, vocation or employment, or any form of vocational or educational training. Provided, however, that "employment" shall not, for the purposes of this article, include membership in any law enforcement agency.

§751. Applicability. The provisions of this article shall apply to any application by any person for a license or employment at any public or private employer, who has previously been convicted of one or more criminal offenses in this state or in any other jurisdiction, and to any license or employment held by any person whose conviction of one or more criminal offenses in this state or in any other jurisdiction preceded such employment or granting of a license, except where a mandatory forfeiture, disability or bar to employment is imposed by law, and has not been removed by an executive pardon, certificate of relief from disabilities or certificate of good conduct. Nothing in this article shall be construed to affect any right an employer may have with respect to an intentional misrepresentation in connection with an application for employment made by a prospective employee or previously made by a current employee.

§752. Unfair discrimination against persons previously convicted of one or more criminal offenses prohibited. No application for any license or employment, and no employment or license held by an individual, to which the provisions of this article are applicable, shall be denied or acted upon adversely by reason of the individual's having been previously convicted of one or more criminal offenses, or by reason of a finding of lack of "good moral character" when such finding is based upon the fact that the individual has previously been convicted of one or more criminal offenses, unless:

(1) There is a direct relationship between one or more of the previous criminal offenses and the specific license or employment sought or held by the individual; or

(2) the issuance or continuation of the license or the granting or continuation of the employment would involve an unreasonable risk to property or to the safety or welfare of specific individuals or the general public.

§753. Factors to be considered concerning a previous criminal conviction; presumption. 1. In making a determination pursuant to section seven hundred fifty-two of this chapter, the public agency or private employer shall consider the following factors:

(a) The public policy of this state, as expressed in this act, to encourage the licensure and employment of persons previously convicted of one or more criminal offenses.

(b) The specific duties and responsibilities necessarily related to the license or employment sought or held by the person.

(c) The bearing, if any, the criminal offense or offenses for which the person was previously convicted will have on his fitness or ability to perform one or more such duties or responsibilities.

(d) The time which has elapsed since the occurrence of the criminal offense or offenses.

(e) The age of the person at the time of occurrence of the criminal offense or offenses.

(f) The seriousness of the offense or offenses.

(g) Any information produced by the person, or produced on his behalf, in regard to his rehabilitation and good conduct.

(h) The legitimate interest of the public agency or private employer in protecting property, and the safety and welfare of specific individuals or the general public.

2. In making a determination pursuant to section seven hundred fifty-two of this chapter, the public agency or private employer shall also give consideration to a certificate of relief from disabilities or a certificate of good conduct issued to the applicant, which certificate shall create a presumption of rehabilitation in regard to the offense or offenses specified therein.

§754. Written statement upon denial of license or employment. At the request of any person previously convicted of one or more criminal offenses who has been denied a license or employment, a public agency or private employer shall provide, within thirty days of a request, a written statement setting forth the reasons for such denial.

§755. Enforcement.

1. In relation to actions by public agencies, the provisions of this article shall be enforceable by a proceeding brought pursuant to article seventy-eight of the civil practice law and rules.

2. In relation to actions by private employers, the provisions of this article shall be enforceable by the division of human rights pursuant to the powers and procedures set forth in article fifteen of the executive law, and, concurrently, by the New York city commission on human rights.

As a MinuteClinic Nurse Practitioner, you will provide accurate assessment, diagnosis, and treatment of common family illnesses for patients 18 months and older. You will have the tools, training and resources to help us with our CVS Health mission of helping people on their path to better health. You will report to the Senior Practice Manager (SPM) and:
• Provide holistic, evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients. This includes education and treatment for pregnancy prevention, sexually transmitted infection (STI) Prevention and safer sex practices.
• Offer, order and administer immunizations and injections (to include but not limited to COVID19, influenza, TB, Depo) and provides evidence-based patient/parent vaccine counseling and education.

• Complete the DOT certification exam within 30 days of employment

Job Summary

The Wellness Associate reports to the Wellness Director and the Wellness Manager and provides services and care to each Resident based on the individual Resident Service Plan.

Wellness Associate Procedure:

Essential Functions:

  • Assists Residents with activities of daily living, including bathing, dressing, eating, toileting, hygiene, and mobility if needed.
  • Maintains an awareness of each Resident’s general state of physical and mental health and reports any changes at once to the Wellness Director.
  • Keeps an inventory of medical supplies on hand and assists with ordering when necessary.
  • Assists with finding shift coverage when unexpected call outs occur.
  • Develops and helps maintain assignments for the Wellness Director.
  • Assists in orienting and on-boarding of new Wellness Associates.
  • Responds to Resident emergencies; provides first-aid and arranges for appropriate medical follow-up.
  • Assists Residents in keeping their units clean, neat, and orderly.
  • Answers calls for assistance and attends to Resident needs promptly.
  • Assists Residents with clothing and laundry needs if instructed to do so.
  • Assists Residents with meal selection, service, and feeding as assigned.
  • Assists with activity program as assigned.
  • Maintains records according to policy.
  • Maintains files as needed.
  • Participates in a minimum of 6 hours in-service education per year.

You Will Be Responsible For:

  • Demonstrates effective assessment skills to ensure quality patient care is provided.
  • Performs and documents accurate initial assessment per unit standard which may include: initial observation, obtaining and interpreting information in terms of the patients' needs, safety, discharge planning, psych/social/cultural, family needs, multi-system physical assessment, patient/family level of knowledge and readiness to learn about disease process and care requirements. Reports appropriate information in a timely manner to responsible physician.
  • Observes and monitors patient lab work for trends and reports abnormal lab tests to the appropriate physician and obtains appropriate interventions.
  • Utilizes clinical data to implement appropriate interventions.
  • Performs and documents ongoing patient assessment which accurately reflects patients' physical and mental status ranging from child to geriatric according to unit standards and patient condition.

What You Will Need:

  • Current Florida Registered Nurse license required or if new graduate, offer contingent on passing NCLEX
  • Basic Life Support (BLS)- (Accredited through American Heart Association) required
  • Graduate of school of nursing with a minimum of an Associate of Science degree or diploma
  • Bachelors (BSN) Degree Preferred
  • National Certification is area of practice preferred
  • Member of national professional organization preferred
  • Previous hospital/acute care experience preferred

Job Summary:

The Registered Nurse is responsible for the delivery of coordinated nursing care for a patient or an assigned group of patients within a patient care unit and for the day-to-day management and supervision of the related clinical activities. The Nurse is responsible for supervising and responsibly directing the activities of various levels of assigned nursing personnel utilizing both professional and supervisory discretion and independent judgment. Responsibilities

About the Position:

 

During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits.

The Matrix Comprehensive Health Assessment helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit.

 

 

Nurse Practitioner - Full Time – Home Risk Assessments

 

Type: Full Time Permanent

 

Location: Home, SNF, Telehealth and other community environments in: Atlanta, GA

 

Hours:  Monday – Friday, 8 to 5:30 PM

*Hours can vary but full time work week expected*

 Salary: Base Pay $80,000 per year + years of experience

+ Additional incentive based on completed assessments

Matrix Provider – What to Expect:

  • Make a difference in people’s lives by conducting Adult/Geriatric Assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols.
  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers and any appropriate ancillary medical personnel as appropriate. Collaborate with Primary Care Physician (PCP) on patient education.  Provide follow-up.
  • A nationwide network of advanced practice providers to build and maintain relationships.
  • Services may be provided in a variety of venues to include; Home Visits, Skilled Facility Visits, Mobile Clinic Visits, Telehealth Visits, and Retail Clinic Visits.

 

 

Qualifications

Must Haves:

  • Master’s Degree required OR commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP licensure in state of practice to include prescription authority or the ability to obtainprescriptive authority.
  • Board certified by the AANP, ANCC or the AACCN in a Matrix approved specialty.
  • Have a current BLS, ACLS or CPR certification.
  • Have a Minimum of 1 year experience as a Nurse Practitioner
  • Have strong computer skills and familiarity with Employee health/medical record software.
  • Possess excellent verbal and written communication skills with patient, clients and colleagues.
  • Be comfortable and flexible with frequent change.

Travel Requirements:

  • Have the ability to travel, a valid state driver license and able to drive a car, proof of adequate automobile insurance coverage for the state of residence.

EXPERIENCE, EDUCATION, LICENSES AND ABILITIES THAT ARE ESSENTIAL CONTRIBUTIONS

Success for the position of a Pharmacy Technician requires that the team member have at least a high school diploma or general education degree (GED), have a current Technician license or certification (in the State where employed if required) and/or hold a National Pharmacy Technician Certification (current or successfully obtain within 12 months of employment), have a minimum of three (3) years of recent experience as a pharmacy technician or equivalent, have data input skills and experience in pharmacy systems, which includes typing skills and familiarity with Windows PC applications.

 

Must have language skills that include the ability to read and interpret prescription documents, policies, procedures and guidelines, interpret documents such as safety rules, operating and maintenance instructions, write routine reports and correspondence, including entries in various computer programs utilized by AHF and AHF Pharmacy, must have the ability to speak effectively with patients, clients and staff members in English (Bi-lingual in English and Spanish highly desirable and Creole in some specific geographies).

 

Must have at least intermediate skills to calculate figures and amounts such as a day’s supply of medications based on the prescribed directions, the ability to calculate dispensing and billing cycles based on days’ supply and billing dates, must have the ability to calculate figures and amounts such as discounts, volume, percentages related to prescription charges and selling prices of goods and services proved by AHF Pharmacy.

Reasoning Skills contribute to patient satisfaction and wellbeing by solving practical problems and dealing with a variety of concrete variables in situations where only limited standardization exists, as well as the ability to interpret a variety of instructions furnished in written, oral, diagram, or scheduled format.
